QUAD A is pleased to have developed and provide a [template policy](https://www.quada.org/hubfs/Resumption-of-Surgery-Policy-Draft-Template.docx) to assist our accredited facilities in preparing for the lifting of restrictions imposed due to the COVID-19 public health emergency. This [policy template](https://www.quada.org/hubfs/Resumption-of-Surgery-Policy-Draft-Template.docx) will require each facility to revise it to meet the individual needs of your facility. To facilitate this effort, QUAD A is providing this document in Word format, with a “company” field that will only need to be entered once. Once you enter your facility’s name in the “company” field located in the title, it should automatically populate throughout the template.
