As of October 13, 2020, at 11am Central Time, many facilities were still non-compliant with Quad A’s 2020 Period 3 PSDR requirements. If you work with multiple Quad A accredited facilities, please sign into the PSDR system and check on your compliant status. If you have come into compliance (including submission of your facility’s cases and any applicable physician override forms) after October 13, 2020, at 11am Central Time then please ignore this reminder. Please note that any staffing changes that occurred during the period may have an impact on your status and you need to communicate with our office to ensure that we are aware of such changes.
PSDR Period 3 relates to cases performed in an Quad A accredited facility between July 1 and September 30. Random case reviews for PSDF Period 3 are due by midnight October 18, 2020 (Quad A has provided a three-day extension over the weekend to encourage compliance). Please make sure your facility’s physician(s) each submit 3 random cases and any sequelae that occurred during the period. If any physicians performed fewer than 3 cases during the period, please submit data for all cases such physician completed and send a completed Physician Exemption Form to the Quad A office.
Quad A will place facilities that remain non-compliant with Period 3 PSDR reporting on probation as of October 19, 2020. Facilities that are placed on probation have 60 calendar days to submit their cases through the online PSDR system and will be charged a penalty fee of $100 per non-compliant provider. Failure to comply with this requirement by the end of the probation term may result in revocation of the facility’s accreditation.
