New Feature in PSDR: Adverse Event Reporting

QUAD A has launched a new reporting feature that lets your facility easily access and export adverse event data directly from the PSDR portal. Use this information to support quality improvement, identify safety trends, and make data-driven decisions that strengthen your patient safety culture. 

QUAD A is pleased to announce a new reporting feature within the Patient Safety Data Reporting (PSDR) system that allows facilities to access and export their adverse event reporting data directly from the portal. This new capability provides facilities with an easy way to review, analyze, and trend adverse event information submitted through PSDR.

Why This Matters

Adverse event reporting is a critical component of QUAD A’s commitment to patient safety and continuous quality improvement. Having ready access to your facility’s adverse event data can help support:

• Quality Assessment and Performance Improvement (QAPI) activities
• Peer Review discussions
• Trend analysis and risk identification
• Governing Body oversight
• Internal patient safety initiatives

Facilities can use the exported data to identify patterns, monitor outcomes, and support data-driven decision-making aimed at improving patient care.

Accessing the Report

Generating the report is simple:

1. Log in to the PSDR portal.
2. Select Reports from the menu on the left side of the screen.
3. Choose PSDR Cases.
4. Review the adverse event reporting data displayed on the screen.
5. Scroll to the bottom of the page and select Export to Excel to download the report.

The exported file allows facilities to analyze their adverse event data based on their individual quality management needs.

Supporting a Culture of Safety

PSDR is more than a reporting requirement—it is a powerful patient safety tool. The collection and analysis of adverse event data enables facilities to identify opportunities for improvement while contributing to one of the largest outpatient patient safety databases available.
