ASC vs. Office-Based Surgery/Office-Based Procedural: How to Determine the Right Accreditation Path for Your Facility

Many patients today prefer to seek surgery and other medical procedures at freestanding ambulatory care centers rather than hospitals. With more options than ever, patients value facilities that prioritize high standards of care and safety. Accreditation is how your facility can stand out from the crowd.

Ambulatory Surgical Center (ASC) Accreditation Program

Ambulatory surgical centers are distinct entities that provide same-day (not exceeding 24 hours) surgical care in a regulated, dedicated setting designed for procedures that do not routinely require hospital admission. Compared to office-based surgery, ASCs generally conduct more complex procedures and anesthesia services.

As part of QUAD A’s Ambulatory Surgical Center (ASC) Accreditation program, your ASC will adhere to regulatory standards for outpatient surgery, pain management, and diagnostic procedures. Meeting and maintaining accreditation requirements set by QUAD A reinforces a culture of continuous compliance, evidence-based decision-making, and operational discipline.

Importantly, accreditation is also an opportunity to qualify for reimbursement for the physician’s or clinician’s work and for the use of the facility and its resources (i.e., operating or procedure room, staff, equipment, supplies, etc.) from the Centers for Medicare and Medicaid Services.

Characteristics:

  • The procedure takes place in a specialized facility that operates exclusively to provide surgical services

  • The procedure is likely complex and may be invasive

  • The procedure may be performed under local anesthesia up to general anesthesia

Eligible Facilities Must Fit the CMS Definition for Ambulatory Surgical Centers:

  • A distinct entity that operates exclusively for the purpose of providing surgical services to patients not requiring hospitalization, and in which the expected duration of services does not exceed 24 hours following an admission

  • The entity must have an agreement with CMS to participate in Medicare as an ASC, and must comply with 42 CFR Part 416 subparts B and C (conditions for coverage).

 

Office-Based Surgery Accreditation Program

QUAD A’s Office-Based Surgery Accreditation program applies to practices performing invasive outpatient surgical procedures in-office under anesthesia.

The accreditation process supports structured patient safety practices, clinical oversight, and operational standards appropriate for office-based surgery and aligned with nationally recognized best practices.

Characteristics:

  • The practice is an office-based setting

  • The procedure likely takes place in a fully equipped operating room or a dedicated suite

  • The procedure is often performed under sedation, but may call for general anesthesia

Office-Based Procedural Accreditation Program

QUAD A’s Office-Based Procedural Accreditation program applies to outpatient facilities that perform outpatient therapeutic, diagnostic, minimally invasive, or minor surgical procedures in-office, under sedation.

The accreditation process supports consistent patient safety practices, appropriate clinical oversight, and operational standards suited to the procedural care environment.

Characteristics:

  • The practice is an office-based setting

  • The procedure likely takes place in a dedicated suite rather than a fully equipped operating room

  • The procedure is therapeutic or diagnostic, or is minimally invasive or minor surgery

  • The procedure is often performed under sedation rather than general anesthesia
